Why Your Hospital Needs Isolated Power Panels

The risk they solve, how they work, and when NFPA 99 requires one

Ask most hospital administrators what keeps them up at night about facility infrastructure, and power reliability is near the top. But there's a more specific risk that's easy to overlook until you understand it: what happens if a single ground fault occurs in an operating room mid-procedure?

The Problem Ordinary Electrical Systems Have in Operating Rooms

In a standard grounded electrical system, a ground fault — current unintentionally finding a path to ground — trips a breaker to protect people and equipment. That's the correct behavior almost everywhere in a building. But in an operating room, where a patient and surgical equipment can form part of that path through conductive fluids, an abrupt power cut mid-procedure isn't a safety feature. It's a new hazard.

How an Isolated Power Panel Actually Works

An isolated power panel (IPP) uses an isolation transformer to separate the operating room's electrical circuit from the building's grounded system. Neither conductor is directly referenced to ground, so a single fault doesn't create the low-resistance path needed to trip a breaker or interrupt power.

Instead, a line isolation monitor (LIM) continuously watches both conductors for a developing fault and sounds an alarm the moment one occurs — typically well before it becomes dangerous. The surgical team gets a clear signal that something needs attention, without power being interrupted mid-procedure. The fault is usually corrected between procedures rather than during one.

When Is an IPP Actually Required?

NFPA 99 requires isolated power systems for "wet procedure locations" — spaces where a patient is likely to come into contact with both electrical equipment and conductive liquid at the same time. In practice, this typically covers:

  • Operating rooms and surgical suites
  • Cardiac catheterization labs
  • Certain critical care and intensive care areas, per facility risk assessment
  • Some obstetric delivery rooms, depending on procedure type

The exact classification depends on your facility's risk assessment and design consultant's determination — not every patient care space needs one, but the spaces that do need one absolutely cannot substitute a standard grounded system instead.

What This Means If You're Planning or Renovating a Facility

If you're building a new operating suite, converting a space to a higher-risk use, or simply inheriting a facility where you're not confident the existing electrical infrastructure matches current code, the isolated power system is not an area to guess on. It requires equipment properly rated for the application (transformer, line isolation monitor, and branch circuit breakers matched to the panel), correct installation and equipotential grounding, and ongoing testing — a LIM that drifts out of calibration defeats the purpose of having one.
